How to claim

A claim can be made through the Fund's Service Centres or through any Nampost office.

MVA Fund claiming procedures.

  • Obtain a MVA Fund claim form from one of the Funds' Service Centres. Claimants can also approach any Nampost office country wide for assistance with completing the form.
  • Complete all applicable paragraphs.
  • Have the doctor who treated the injured person immediately after the accident complete the medical report section of the form.
  • The doctor must sign the form and endorse it with his/her practice stamp.
  • Claimants must sign the form in the presence of two witnesses.
  • The two witnesses must also sign the form in the presence of the claimant.
  • Always attach a police report of the accident.
  • Once all documents are collected and completed, lodged the claim with the Fund, either by registered post or hand delivery to the MVA Fund offices or any Nampost official.

The claim is subjected to various stages:

1. What types of claim can you lodge with the Fund?

Funeral Grant; hospital and medical expenses; loss of earnings; loss of support if a breadwinner died in the accident as well as Injury Grant.


2. Who may claim?

  • A person who was injured;
  • A dependent of a deceased person as well as a person who has incurred funeral expenses of a deceased person;
  • A driver who was not at fault can also claim;

3. What is the time within which you can lodge a claim?

The time period within which a person can claim for re-imbursement and benefits from the Fund is one year under the Act. The Fund also has the right to serve a notice on a claimant to submit a claim within 90 days of the accident having occurred.

If a person injured in an accident then fails to claim within the set period of time (90 days), the claim will not be considered.



4. What documents need to be submitted?

Any document which can proof that the claimant sustained injuries as a result of a motor vehicle accident.

In respect of a Loss of support claim by dependants of a deceased:

  • A duly completed claim form in respect of each dependant;
  • Full birth certificates of each dependant;
  • Certified copy of claimant's ID;
  • Letter from the employer of the deceased clearly stating their period of employment and net salary at the time of death accompanied by a salary certificate;
  • Marriage certificate;
  • If the indigent parents (he/she has no other means of support) of the deceased submit a claim, then proof of indigence (a detailed Affidavit) must accompany the claim;
  • Details of children that were dependent on the deceased;
  • Accident Report (Pol 66), Witness Statements, Drivers Statement, Investigating Officers Statement Sketch plan and key;
  • Copy of tax certificate;
  • The claimant's statement under oath.

In respect of General Damages/Injury Grant:

  • Certified copy of claimant's ID;
  • Accident Report (Pol 66), Witness Statements, Drivers Statement, Investigating Officers Statement Sketch Plan & Key;
  • Claimant's Affidavit (statement) clearly stating where and how the accident occurred and giving the details of the vehicle that caused the accident;
  • Medical records of the claimant;
  • Certified copy of the claimant's health passport;
  • All medical receipts (if any);
  • If scars are visible on the body, then photographs of scars and claimants face should accompany the claim;
  • Letter from the claimant's employer stating the exact period the claimant was absent from duty as a result of the accident and exact salary lost as a result thereof;
  • If the claimant is a minor, a full birth certificate of the minor child is required.

In respect of Funeral Expenses/Funeral Grant:

  • Duly completed MVAF 1 Claim form;
  • Death Certificate;
  • Post mortem report;
  • Certified copies of the deceased and claimants ID documents/full birth certificate/marriage certificate or letter of the executor.

5. Where can you get a claim form?

You can get a claim form at the MVA Service Centres in Windhoek and Ongwediva or at any Nampost outlet where the MVA does not have an office.

6. What is the waiting period to be compensated?


This depends on the type of claim. Once all completed forms have been received, funeral claims can be settled within 1 day and other claims within 30 days.
